Before you begin, take a moment to reflect on who you are today and who you want to become. This form is designed to help you think about your present identity—your interests, values, and goals—and set intentions for your future. In the “My Present” section, describe what motivates you right now and how you’re growing. In the “My Vision” section, imagine where you see yourself in 5–10 years—personally, academically, or professionally. A sample has been filled out for you as a reference, but this form is personal, so write in a way that feels true to you. Be honest, thoughtful, and don’t worry about having perfect answers—this is about discovering your path.
